Through the Home & Community-based Services Program at our affiliate Full Life Care, we offer a range of services uniquely designed to make an impact where it counts. We work with vulnerable older adults to achieve housing stability, directly preventing homelessness and avoiding last-resort supports like costly long-term care. Our programs work upstream to save taxpayers money and support our community.

Community Transitions Program
Full Life Care is working one-on-one with older adults at risk of losing their housing, making an impact where it counts by preventing homelessness and avoiding last-resort supports like institutional care.
Foundational Community Supports
Full Life’s Foundational Community Supports’ housing case management team works to help eligible clients find and maintain independent housing, working upstream to save tax payers money and support the most at-risk older adults in our community. The program’s impact and ability to provide critical supports is facing increasing pressures with funding and we need your help to continue.
